Our Leadership

Joseph Webb, D.Sc., MSHA, FACHE, is a nationally recognized healthcare leader and population health strategist. With over thirty years of experience in healthcare delivery and population health management innovation, Dr. Webb specializes in helping communities turn evidence-based healthcare frameworks into practical population health strategies that improve outcomes, enhance community well-being, and lower costs while maintaining sustainability. During his career, Dr. Webb has led three hospital turnarounds, one in each sector of the healthcare industry: private for-profit, private not-for-profit, and public hospitals.

Through Precision Healthcare Consulting, Dr. Webb advises healthcare organizations, municipalities, and community coalitions on implementing coordinated population health management strategies. His work emphasizes aligning healthcare providers, public health agencies, social services, faith‑based organizations, and community partners to address the underlying causes of health outcomes and improve access to care, with a focus on chronic disease management.

A central focus of Dr. Webb’s work is community‑driven population health management. In 2015, he developed the Webb Health Equity & Sustainability Model™, an integrated framework designed to operationalize population health management by utilizing a closed-loop model that combines the Chronic Care Model, Patient‑Centered Medical Home principles, Epidemiological Management, the Donabedian Quality Model, and Community Health Needs Assessments. This model helps communities coordinate healthcare resources while addressing social determinants of health and advancing the strategic goal of achieving the Triple Aim of Healthcare: improved outcomes, improved care experience, and lower costs.

With Precision Healthcare Consulting, Dr. Webb continues his work by serving on boards, providing advisory services, and consulting with communities and organizations to develop coordinated population health management strategies that align local healthcare resources, community organizations, and civic leaders around measurable improvements in health outcomes. His work demonstrates that population health management, when used as an intervention, can transform fragmented healthcare systems into coordinated networks that improve quality of life, strengthen communities, and promote economic growth.
